This notice explains how MedXplorer360 handles information you submit through the request form on this website.

What we collect

Only what you enter: your name, phone number, optional email, city, hospital or clinic, cancer type, diagnosis, test of interest, your question and your preferred contact method. We ask you not to enter ID numbers or full medical history.

Why we collect it

To manage the prebooking list, to respond to your request, to discuss which test may be appropriate with you and your treating doctor, and to arrange next steps.

Who can see it

Members of the MedXplorer360 team who handle requests. If a request proceeds to testing, information needed for the test is shared with the laboratory and the treating doctor, and you will be told before that happens.

Retention and your choices
